I acknowledge that services offered by Marcia’s Looks Salon including but not limited to haircuts, coloring, chemical treatments, hair extensions, and other beauty procedures, carry certain inherent risks. These may include allergic reactions, skin or scalp irritation, hair damage or breakage, or other unforeseen side effects. I voluntarily accept these risks and release the salon, its employees, contractors, and affiliates from any liability for injuries, damages, or losses that may occur as a result of these services.

The Client agrees to: Provide accurate and complete information regarding my hair and skin condition, medical history, allergies, and past treatments. Follow all aftercare and maintenance instructions provided by the salon. Immediately notify the salon of any adverse reactions or post-service concerns.

I confirm that I have disclosed all known allergies, sensitivities, and relevant health conditions that could affect my service. I understand that failure to provide this information increases the risk of unwanted reactions, for which Marcia’s Looks Salon will not be held responsible.

While our team is committed to delivering exceptional results, I understand that individual outcomes may vary due to factors like hair texture, color history, chemical response, and home care. The salon does not guarantee exact results. Any adjustments or refunds are provided solely at the discretion of the salon and according to its established policies.

I understand that my personal items are my sole responsibility. The salon is not liable for any lost, stolen, or damaged belongings during my visit.

I grant permission for Marcia’s Looks Salon to capture photos and/or videos during or after my service for marketing and promotional purposes (including social media, website, and printed material). I understand that no compensation is owed for the use of such media. If you prefer not to be photographed or recorded, please notify your stylist before your service begins. If a photo or video is posted before an opt-out is requested, it will remain published.

A non-refundable $25 deposit is required for regular bookings. A 50% deposit is required for services lasting over three hours or totaling more than $300. Deposits will be forfeited if I cancel with less than 24 hours’ notice before the scheduled appointment. In case of same day appointments a minimun of four hours is require to cancel or reschedule.

By signing this form, I release and discharge Marcia’s Looks Salon, its owners, employees, contractors, and affiliates from all claims or causes of action arising out of the services provided, including personal injury, property damage, or financial loss.

This agreement shall be governed by the laws of the State of Florida and is binding upon myself, my heirs, legal representatives, and assigns. I affirm that I am at least 18 years of age or have received the consent of a parent or legal guardian to receive services.
